We work with the young people in the facilities on reintegration goals from the very beginning of their time at the facilities. We meet with them and with their families within 30 days of receipt of the referral. SRP Staff follow up with subsequent follow sessions with the youth every 60 days. Meanwhile, staff in their home communities, are working to gather resources to help with applications and goal setting early on. Approximately 90 days prior to discharge the SRP staff coordinate an Discharge Planning Meeting including: the youth, the staff at the facility, the youth's probation officer, the youth's community case manager (CCM), and any other service providers that will be involved in the youth's successful community reintegration. 30 days prior to discharge an Individual Service Plan (ISP) is developed that summarizes the goals everyone in the Discharge planning meeting agreed to.
