Once the youth discharges from the facility the youth's Community Case Manager (CCM) will either meet them at their discharge Court hearing OR will make phone contact within 24 hours and face to face contact within 48 hours of the youth being home. For the first month the youth is in the community the CCM and the youth will meet for a minimum of two times a week face to face and will have at least 1 phone contact per week to work on goals identified in the ISP. After the first month, the intensity of services may vary depending on the youth's needs. We generally work with youth for four months in the community after discharge.
